FloridaUCC, Inc. will accept and process other forms as Unapproved Forms only if the forms contain the same fields as the most recent corresponding Florida UCC-1, UCC-1 Addendum, UCC-1 Additional Party, UCC-3 Addendum, and UCC-3 Additional Party forms approved by the Florida Secretary of State.  Forms approved by the Florida Secretary of State prior to December 2001 will not be accepted for processing.  See the Fees section of this web site for fee information related to "Approved Florida Forms" and "Unapproved Florida Forms".

If you have difficulty downloading the forms, you can order UCC forms directly from a Forms Supplier. UCC forms are not available from Image API, Inc. or FLORIDAUCC, Inc..

Note: UCC1 and UCC3 addendum and additional party forms are optional and are considered additional pages when submitted. There is a $3.00/page fee for additional pages.
